You are invited to participate in the HPV Vaccine Study led by Dr. Shillpa Naavaal from the VCU School of Dentistry. She and her team are conducting this study to better understand the practices, perceptions, and provider experiences related to the Human Papillomavirus (HPV) vaccine.

The VDA leadership supports Dr. Naavaal in her research and would encourage you to provide your input on this important topic. This brief survey will take approximately 10 minutes to complete. Your participation is voluntary. Participants who complete the survey will have a chance to enter in a gift card raffle and win one of twenty $50 gift cards.
