As of January 8, Peninsula Health District is actively reaching out to dental practices to arrange vaccine appointments. Please note, they can only contact you if you have already submitted your Vaccine Request Form via online link or emailed request form, and have your practice name and contact information. After next Wednesday, they will switch to an online appointment platform. Please ensure that your practice’s Point of Contact will be on the lookout for either a phone call or email to access the online appointment platform.
